Here are to be found the most frequently used forms and documents in the JPT Band & Orchestra Program.  These documents come in Microsoft Word or Adobe PDF format.  If you have Microsoft Word or Adobe Reader, all you have to do is click the form and it will open for you.  The added benefit of Microsoft Word is that you can download it into your computer and type into the form, although some of the fonts may appear differently on your computer.  For those who are interested, the fonts used on most of these documents can be downloaded from this Zip file.  Once opened, move the fonts into your C:/Windows/Fonts directory to be able to see all the fonts correctly.  If you have any problems with these links, please contact the webmaster.

Adjudication Sheet    Word Format 
This document is used by all of our band & orchestra students to evaluate bands and orchestras at our District Concert Assessments.  Each student must do two evaluations.  For those who for some reason cannot do the evaluations on the day your ensemble performs, and for those who wish to evaluate extra ensembles for extra credit, we provide these adjudication sheets online.

Band and Orchestra Handbook (including Participation Contract) PDF Format (updated 08/29/07) 
This is the Handbook that governs all facets of the JPT Band & Orchestra Program.  It is a wealth of information on our rules and policies, and is required reading of all members and parents.  At the back of the Handbook is a Participation Contract that must be filled out and returned by the end of the first week of classes.  Please familiarize yourself with the Handbook to avoid any misunderstandings.

One of our biggest fundraisers is our Football Program.  A certain percentage (25%) of each sale is placed into the student's account.  The more ads you sell, the more money in your account.  Every student should try to sell at least one ad.
